They need to ship updates provided to them by AOSP and the SoC manufacturer. It's not as if they're creating the updates themselves. It's quite easy to ship these. As long as they don't make non-trivial changes to the OS, it's also not very hard to ship the major updates either.

I think the monthly security updates can be considered easy even for vendors with substantial changes. Making more substantial changes implies having more resources. Most don't make enough changes to have many (or any) conflicts / issues with the monthly security updates.

Major upgrades are a huge undertaking if they make substantial changes but it's something they take on when they decide to do it. There are 3 years of security updates for each major release though, and they could easily provide those even without doing the major upgrades.
