They need to ship updates provided to them by AOSP and the SoC manufacturer. It's not as if they're creating the updates themselves. It's quite easy to ship these. As long as they don't make non-trivial changes to the OS, it's also not very hard to ship the major updates either.

I think the monthly security updates can be considered easy even for vendors with substantial changes. Making more substantial changes implies having more resources. Most don't make enough changes to have many (or any) conflicts / issues with the monthly security updates.

Major releases do have substantial privacy and security improvements and are quite important for improving the platform for app developers, but security updates are largely separate. Even if they only did 1 major upgrade, they could do 4 years of security updates for it.
