They need to ship updates provided to them by AOSP and the SoC manufacturer. It's not as if they're creating the updates themselves. It's quite easy to ship these. As long as they don't make non-trivial changes to the OS, it's also not very hard to ship the major updates either.

OEMs choose to make massive changes to the OS. They could save a lot of money by using AOSP with only resource overlays and replacements for some of the apps like Pixels. It's their choice to make the major version upgrades extremely difficult since they have to port it all over.

I wouldn't go as far as to say it's "easy" due to the fact most are making non-trivial changes, but it is certainly easier than having to do it themselves and should be an expected requirement.

I think the monthly security updates can be considered easy even for vendors with substantial changes. Making more substantial changes implies having more resources. Most don't make enough changes to have many (or any) conflicts / issues with the monthly security updates.
