There are many old and unhelpful stereotypes in medicines. The burly orthopaedic surgeon ignoring everything but the bone, the pipe-smoking, cardigan-wearing psychiatrist and the jack-of-all-trades emergency doctor...👉

Because they are not all measuring the same thing and there is no one thing that has most impact... PUMA study will report shortly and had relevance to paediatric and adult practice:

Doublechecked guideline - diagnosis of DKA now mirrors ISPAD's with pH <7.3 OR bicarb <15 AND ketones >3 (note the 'or' for pH / bicarb)

Ultimately I have to agree with : “Sick patients with DKA will continue to need careful monitoring regardless of what fluid regime they are on.” This, for sure.

Lots of biases/issues with twitter polls but I’m not convinced a load of Russian ‘bots are interested in management of head injury. Open over weekend for those who’ve not responded yet. Fascinating divisions here…

This is very true. I find it much harder to be a good supervisor than a good researcher. I think supervision is an achilles heel of many clinical academics

Bordering on Reductio ad Absurdum BUT defines a 3 vomit rule & then says if a further vomit a CT scan is necessary. Clearly there is clinician interpretation needed but this is what the algorithm says. It’s pathophysiological rubbish…

Brief clarification. One should not ignore isolated vomiting after minor head trauma in children. Rather, observe for several hours in ED for worsening or improvement. Observation will help decision-making regarding discharge without CT versus CT.
