Fascinating findings & discussion, esp. that differences in other studies may be driven by inconsistent thresholds for AM surge & lack of adjustment for 24hr BP. Curious if any literature supports higher risk in Asians, accounting for those factors (per prior talks at meetings)?

I have always questioned whether morning surge is predictive for CVD among those who have a high prevalence of hypertension during sleep. The higher the asleep BP the less the morning surge. We examined this question in the Jackson Heart Study.

Its incredible that this paper didn't receive more press- "afterload reduction" doesn't help in acute HF. Given continued negative trials, why are we fixated on the concept? Comprehensive Vasodilation vs Usual Care in AHF

Databases originating from "real-life" registries need accurate data check and cleaning. We manage a large ABPM registry and do this periodically while the database is growing. You must also accept to throw away a lot of rubbish data ;-)
