"if more A, more B" can be interpreted as predictive (P) or as causal (C), and as all-things-considered (ATC) or as holding all-else-equal (AEE). Which is usually the intended interpretation?

Exactly that - the ambiguity is known so people will make an effort to make it clear which they're implying if they're writing / speaking honestly.
