Why is it unexpected? Since they lower deaths from heart disease but *don't* lower all cause mortality logically they have to raise other causes of death and if it's causing deaths it's also causing those same complications at a lower level.

Whether they lower heart disease deaths I think is pure speculation. The only evidence we have for this is studies published by the pharmaceutical industry where independent scientists aren't allowed access to the raw data. I suspect at best they just delay the event.
