The timings of the FH second stage burns seems to indicate that the first two burns will insert the stage into a standard GTO, then it will cruise to 36000 km high and the Earth escape burn will go from apogee (?). (paging @planet4589 et al.)

Normally you'd want to do the escape burn at perigee.
Indeed; another possibility is for the stage to enter a 6 hours period elliptical orbit after SECO-2.

The document to which you linked is not clear on this, but that'd be my guess. Creative people can come up with weird orbits that might accomplish the same thing, but that would be simplest.
