Most worryingly it would infer that Ariane V was off trajectory from launch, and the range safety officer is just fine with a launch 20deg off nominal flying over downtown Korou.

I'm surprised too, although some people are trying to make the case that perhaps the RSO didn't push the button while the flight is still barely within limits and clearly within control.
BTW, range safety at Kourou is provided by @CNES.

Finally, two questions remain: 1) Why did Ariane go the wrong azimuth? Software? Sensor? 2) Why did it go apparently unnoticed so long?

The first part I care significantly less about than the second. The graphics showed it off course, yet the called it as a nominal flight until staging. That’s worrying
