I expect the LM-3B one to be confirmed by the Indonesian customer or their insurers later. One would also wonder if this also caused the LM-7A failure in March since people seems to be thinking of stage separation problems.

Nit on LM-3B failure: some reports earlier indicate that one of the 3rd stage engines failed. Maybe the ignitor is inside one of the 3rd stage engines instead - that page isn't explicit on this.
Yes, they implied that LM-3B's issue was related to manufacturing quality of some pyrotechnics onboard but (I think) it's not referring to specifically failure of the ignitors for retro-firing stage separation rockets between 2nd and 3rd stage of 3B.
Seems like it's just one of the potential weak spots on LM-4B Y45 that was reinforced with added redundancy, along with more checks of pyrotechnics throughout the LM-4B rocket.
I'm more inclined toward the hypothesis of an ignition failure in one of the YF-75 engines on the 3rd stage.
