Apparently the reasons of failure for 2 recent Long March rockets flights - not yet known till now - have turned up in a tender for insurance of the future launch of ZY-3-03! The 2 involved are Long March 4C/YG-33 on May 22 last year and Long March 3B/PALAPA-N1 this April 9.
According to this last year's LM-4C failure was due to "improper welding of 3rd stage engine components", which was not found pre-launch. It mentioned 3rd stage engine parts welding specifications and parameters have been revised, engines inspected & have since flown 5 times.
The failure reason for the April LM-3B was not explicitly mentioned, but it's apparently due to stage separation retro-fire rocket ignitor failure (must have been during 2nd-3rd stage separation per official reports). Ignitors were re-checked & doubled in number on other rockets.
I expect the LM-3B one to be confirmed by the Indonesian customer or their insurers later. One would also wonder if this also caused the LM-7A failure in March since people seems to be thinking of stage separation problems.

Nit on LM-3B failure: some reports earlier indicate that one of the 3rd stage engines failed. Maybe the ignitor is inside one of the 3rd stage engines instead - that page isn't explicit on this.
