If the engines are the culprit, the YF-115 RP-1 engine is only used on the LM-6 & LM-7 2nd stages; only around 2 LM-6 flights are plannes this year and the LM-7 isn't going to fly (at least we know of) until after their space station modules start flying.https://en.wikipedia.org/wiki/YF-115

But there are also chances that the problem is structural, attitude control, GNC or pressurization related. The LM-7A's 2nd stage is based on, but much shortened from that of the LM-7's, so chances are that if the cause is from these the problem would be unique to the LM-7A.

The proximity of the timing of these events would, of course, also raise eyebrows on whether the stage separation went wrong and re-contact occurred. Videos of the 2018 Soyuz MS-10 failure and the September 2007 Proton failure are probably useful for comparisons.
