So that probably points more to a loss of thrust during the 2nd stage burn (casing burn-through?) or loss of attitude control, instead of failure of ignition.

Checking back on the mission, the Vega 2nd stage Z23 motor ignites at T+1:55. Acceleration seems normal (at least within the normal range as I remember for previous Vega launches) until around T+2:12 at around 2.16 km/s, then stopped.

Looking at the video (https://www.youtube.com/watch?v=jJd7-0GDaBU …) we hear « Separation du P80 » at 1:58 (and we see the flame out on the video) then « allumage du Zefiro 23 » at 2:03, but nothing shows on the screen and the velocity starts decreasing at 2:15. So I'm unsure if the Z23 ignited at all
