The DIV M actually flew 3 times: DSCS-A3 (11 March 2003) DSCS-B6 (29 August 2003) DMSP-F17 (4 November 2006, from Vandenberg) IIRC DIV M was to be the baseline version a la AV 401 but the RS-68 performance fell short and most payloads require the M+(4,2) onwards.
I just take a look at the T/W ratio from http://www.spacelaunchreport.com/delta4.html and the Delta IV M comes at around 1.19 - low, but still somehow beats out the Atlas V 401 at ~1.17!
