I was reviewing the Soyuz MS-10 launch. Interestingly even the LD inside the bunker wasn't aware of the immediate situation and seemed to had nominal call outs for about 20 s after the incident, then simply counted down seconds without comment! (1/2)https://youtu.be/CnQlj73NIm4
In comparison I remembered at least one Proton failure (Express AM4R in 2014 IIRC) where the failure call out was almost immediate. (2/2)

Worst call evah was during an American live radio broadcast of SLS-51L when the commentator happily shouted"excellent video!" as the vehicle disintegrated ... but NASA's "major malfunction" description of its loss a bit later was also something.

Second place goes to the official live commentary of that Delta 2 GPS launch in 1997 that failed in a spectacular explosion seconds after lift-off, forcing spectators to run for cover: "we've had an anomaly ..."

reminds of a failed Soyuz rocket launch in 1996 when the commentator continued to read "N seconds into flight, all is nominal" even though rocket had already exploded by then :~)
