1. Does guardian activate once per turn or every block, like when blocking Cataphracts of the Sand twice in a turn with the save creature?

2. When using Chilling Fog against a creature with relentless, does the creature not get frozen because it's not exhausted or does the freezing cause the creature to get exhausted or does it freeze in an unexhausted state only preventing it from blocking next turn? ...

The creature gains a Freeze marker (thus preventing it from refreshing as normal during the controller’s next Refresh phase), but since it’s not exhausted from attacking it functionally has no effect. If it becomes exhausted via another effect the Freeze will occur as normal.

Nope. The only thing a Freeze token does is make the card not refresh as normal during the Refresh phase, then all Freeze tokens on that card are removed. As long as the card isn’t exhausted, Freeze doesn’t have any effect (but will still be removed during the Refresh phase)

That’s one of the benefits of Relentless when attacking into something like Chilling Fog (as long as your opponent doesn’t have other ways to exhaust your Relentless creature, of course :) )
