This is the biggest problem with Obamacare IMHO, mandating the overhead of the private insurance layer. Insurance ≠ healthcare.

The Netherlands make it work, but it's so heavily regulated that I doubt U.S. insurers would be happy with their model.

How much a government spends will impact prices. It's consistent globally. So no gov. Spending would actually decrease prices further
