If they commit violent crime, it's a police matter, sure. Refusing treatment? Offer again, offer better, knowing some will always refuse.

Makes sense. In Portugal, an addict busted for a violent offense would be referred to treatment run by Dept of Health, not Dept of Justice.

Yes - Portugal uses "dissuasion committees" (sounds Orwellian, but isn't) which can waive criminal penalties if people seek treatment.
