also wonder how much of the difference is attributable to use & access to care (e.g. the sicker people are, the higher their use/spending)

No doubt accounts for some the difference. Americans less active, more obese than OECD peers, no? But only explains a point or two.

Well, I suppose more so the prevalence of chronic illnesses, overall. (car crashes, gun violence, complications from alcohol/drugs too?).
