      I mean, in a sense those aren't crimes because they don't result from immediately deliberate actions by conflict actors

      and you're right, they and other second-order effects of conflict--displacement, disease, malnutrition, etc cause most deaths.

      but they're often still the result of decisions made by those who are willing to accept those indirect costs of their actions

      I don't think there's an IHL framework for that--and sometimes the costs may be worth bearing (say, beating the Nazis)
