    This Medicaid bargain of paying best price in return for an open formulary is central to drug reimbursement. While I'm sensitive to altering only part of bargain -- we have been seemingly shifted efficacy threshold at the same time, thus changing consequences of open formulary.

    The important thing here is to only eliminate out of pocket costs for patients on at least one product in a therapeutic category. Where there are multiple potential products, cost-sharing can remain an important negotiation tool to lower over all drug spending.
