I have responded to medical emergencies on flights, including American. The first thing staff ask is do we need to land. If so, it takes time to get clearance, a emergency flight path and a runway.

There is the emergency, there is also safety of all, and the control towers do a great job helping. Frankly, a pulmonary embolism has a high death rate. Factors like sitting for long periods, dehydration, genetics, drugs like the Pill, increase risk for a PE.

I found the medical kit on American better than most, you can resuscitate using one. I cannot comment on that flight, just give my experience.

The other point here is that the doc on the plane did not know the cause of the patient collapse. Could be heart issye, stroke, embolism, overdose, allergic reaction, etc. Doc works on CPR, can talk to ER doc on ground. But not all can be saved.
