Mistakes happen, even with the best leaders, because they're still human. Good leaders own those mistakes and work to correct them, rather than throwing subordinates under the bus.

People who do their jobs right don't allow mistakes to happen... Nobody is questioning this persons integrity. But clearly when it comes to nuclear threat warnings we need more competent people handling it.

For some reason, I find it easier to have confidence in people who immediately own any mistake in their jurisdiction than people who don't appear to make mistakes.

My concern is not the incident, but the length of time for the retraction. Didn’t the mistake-maker & supervisors get the warning asap like the rest of the state? Didn’t they think right there “oh crap? Which button did I push?”
