For me it is about 7 Doctors from one unit in one Hospital lying and covering each other’s failings. They just feed off each other’s lies. For me it is about them finding out what is at the end of the path they thought why would put our family on. University Hospital Bristol

That’s a known. I found up to 3 errors/sentence in my hospital notes. Apparently product of how typing up notes takes place; multiple people input, often from scribbled notes. Some were ironic: Bupa Dr claimed I ran 17mi 3x /week. I’d run 19mi, 17 logged, 3 days before DVT onset.

Well I wish my GP would have this attitude to the incorrect records we have pointed out instead of blank refusal putting my son’s care at risk. Well done

Similar issue w/ me. In the end, I accessed copies of all my records: each provider charges £50, so it's expensive. Then retain a copy. I take a copy of relevant records along with any referral request in writing, & I retain my own file copy. It often fails to be effective, tho'.

Often, good clinicians, confident of a high standard of care, will share letters, info., in real time. That's not historically routine; but there are examples of good practice. W/ funding cuts, NHS may wish to email or digitally share, to avoid printing charges: that's great too!

Practice refusing to share letters and records which would help overcome disabilities of memory loss and hearing impairment. Shocking lack of empathy and care. Now served with SAR but they insist on printing refusing to email.

It can still be really positive. You can file in a lever arch with dividers; helpful for checkups. You can photograph key documents to store securely and share with your GP or other specialist care providers in appointments. It’ll help your GP too, if you are familiar w/ records.

NHS Ombudsman said up to 3 errors/sentence is totally normal. Explained they happen through multiple people involved typing up, approving, hospital notes. It’s weird; my dad used to write technical reports. Dad just dictated report, handed the recording to a skilled touch typist.

My, now retired, new GP is the only GP that I’ve seen do that. Thinking, one of his practice partners started doing it also. It’s a brilliant option if skilled touch typists are employed, available. It must reduce the opportunity for introducing avoidable communication errors?
