      My biggest problem in all this is preventing anything like this happening to another Jack by learning the right lessons and taking the evidence from around the world about what works in ensuring the highest of safety standards for all patients, no matter what their background.

      Will your aim be achieved by Dr Bawa-Garba's appeal(s) and potential reinstatement? By a potential retrial (or whatever is planned) - is anyone really sure, without seeing the trial transcript that she was not 'truly exceptionally bad?'

      Would all the doctors subscribing to the #iamhadiza have made all 21+ mistakes that she did (I haven't seen a list, has anyone?). Is 21+ errors acceptable to other doctors in the light of her working conditions that day? Would they also be acceptable to patients?

      We don't know what they were. May have been mostly trivial errors obvious only on later scrutiny. Likely to be errors of omission rather than commission in stressed system. You're asking others to speculate Kate.

      How is suggesting they 'may have been mostly trivial errors obvious only on later scrutiny', or that they were 'likely errors of omission rather than commission in stressed system' any less speculative, David? And that's not just aimed at you, btw.

      The word ‘may’ is critical. If we accept that errors vary dramatically in significance, then “21 errors” is meaningless. Only by knowing exactly what those errors were (or having them explained by a trusted expert) can we answer Kate’s question.

      I agree with the last part - hence my desire for the transcript to be published. I think the word 'may' simply helps distinguish speculation from assumption in this instance.

      I agree. The 'may' here is what avoids it being assumption and introduces the uncertainty required to instead be viewed as just speculation.

          Kate’s speculation is an established fact! We only need the transcript if we are attempting a complete retrial. We can identify and discuss general issues without needing to reach a verdict on any particular case.

          If attempts should emerge to try and change something about the law as a result of this case, then I do think we need the transcript. Retrial or not.

          I don’t think there’s any need to change the law. BG’s defense relied on human factors, but awareness of human factors science remains poor.
