      Me too! Many people have waited years for a moment of this sort to arrive. The conversation finally has real meaning and is being taken seriously, rather than being dismissed with platitudes. We all have a responsibility to ensure the opportunity to improve things isn't lost now.

      It would be physically & intellectually impossible for one pilot to fly 2 planes It is physically possible for a paediatric registrar to care for 2 very ill children simultaneously but it is an intellectual impossibility & dangerous as was demonstrated in this tragic case

      It's also physically possible to be grossly negligent in your personal actions, regardless of the shortcomings of the environment you've been placed in. Let's not forget none of have that transcript yet, before we start deciding what this case does or doesn't demonstrate.

      Everybody has suddenly seemed very keen on healthcare/aviation comparisons of late (following a startling change of heart recently) so how does that work here? If we tolerated every pilot having such day every 10 years as 'understandable', how many plane crashes would there be?

      Far less than that number, because of the redundancy in that system. Little redundancy in medicine.

      Why is there little redundancy in medicine? If I had a pound for every time I heard the NHS tell us that lessons have been learned, I could cover the NHS budget shortfall for you myself. What happened to all those lessons?

      Redundancy/backup means not making mistakes because another spots your human error despite checklists/guidelines. It doesn't matter how many times you tinker with checklists/guidelines, you'll have a far smaller effect on safety than having two people backing each other up.

      But like any regulated environment, there must be adequate supervision, no? Somebody with eyes on every now and then to make sure all is well?

          My belief is that most organizations (across most industries) don’t offer meaningful supervision to supervisors. I’d be delighted to be proven wrong!

          I can only speak knowledgeably of 1 regulated industry and that was pretty good up to 2nd line, but fell fairly flat once you ventured higher. All too common to find specific supervision skills are not well trained at best, wrongfully assumed to be obvious at worst.
