          Absolutely. "Lessons learnt" is a popular smoke screen tactic. Also an admission of guilt... "we were a bit naughty but now we have ticked some boxes so that's ok"

          And don't forget to call it 'historic'. Means it doesn't matter any more. All shiny and new now. Dawned on me how selective this observation is too. Prosecution lawyer may prevent them, but defence lawyer wouldn't have done so, if it helped. Creates a misleading impression, imho.

          Oh dear this sounds familier in my case with the damming medical expert report from Dr and nursing medical expert report withheld from inquest but defendants saw Dr's medical expert report they wanted to ajourn inquest because of the report
