I expect you will find #learningfromdeaths is going to be a time limited exercise, publish best practice recommendations then move on.

Umm.. Well, the Parliament Heath select committee into the NHS complaints procedure was submissions by December 2010. I sent mass of evidence to prove complaints procedure untenable = nochange. I've a feeling next will be investigation into Inquests also untenable!

Inquests definitely need investigation. With regional supertrusts becoming more prevalent and transfers being more common deaths still being investigated by coroners local to where person lived leaves more holes in system than Swiss cheese. Maybe IME process will help this?

The idea is to not need an inquiry into avoidable deaths but have an ongoing process that investigates and learns on an ongoing basis. Leaves a lot of us so called ‘historic cases’ unresolved though. Anyone ever counted us up? Unresolved cases and number of years..

Sad thing here is, (just off the top of my head), if we put those like Derek/Joan Bye,
@willcpowell,@ann_poppy &@tomsanguish together, the years the rest of us have spent combined, would probably be dwarfed. It's a list the NHS should be thoroughly ashamed of.2 replies 2 retweets 3 likes -

ZHH These + a few more were the people who taught me early in 2009 what a hard fight we would have over

Our heroic, if unwitting, pioneers. Few probably realise how much we owe to them. Without their help and the chance to learn from their personal ordeals, I don't know how far I would've got. I'll be forever grateful.
