No pain relief, then? 'Perfectly naturally' doesn't mean 'well' and 'naturally occurring' doesn't mean 'not poisonous'. There is not much 'common sense' within NHS protocols - and 'we all have different versions re 'common sense''(i,e it often isn't 'common'!).

If the patient requests it, a dose which effectively relieves the pain: where on earth do you get 'a minimal dose of a gentle painkiller' from? Pain does not 'have to be relieved' - it should be the patient's decision if capacitous [and very complex to discuss if not capacitous!}

Yes, I think nurses in particular sometimes think that pain-relief/sedation can be administered without consent: the solution to most EoL problems, is to 'get ongoing consent' (or 'ongoing properly-applied MCA best-interests decision-making). 1/2 I honestly think, there is an issue: HCPs - especially I suspect nurses - tend to be focusing on 'the treatment pathway' and they don't think of 'ongoing consent for every individual 'action'' - but during EoL 'repeatedly checking for consent' would bring to light the 'this

Yes, I'm increasingly aware of the difficulty that any suggestion of a lack of capacity introduces into the mix. It wasn't an issue I faced, personally. But it can be hard enough to ensure autonomy is respected when that's not a consideration, so I appreciate your point.

I was vaguely aware of concerns while researching, but I was only too happy to skip extra research I didn't need. What I've read of yours so far has raised my awareness considerably. Just enough to help me understand the scale of my ignorance. Need to go back over what I skipped!
