I had agreement from head of info governance last january that I cld have a copy of every appointment records with my care co-ordinator at next appointment and could have additions made as an edit to the contemporaneous record. So far, two copies out of numerous meetings.

I like your thinking. Interesting that they've failed to deliver, despite agreeing to it. Why, one wonders? Do you record your consultations?

Thinking for positive future*. Positive interactions happen when a professional openly shares notes with the patient. Allows for open effective, communication. This does happen sometimes. *Real-time electronic secure sharing via an automated log in (inter-)national (PDF) system?
