Yeah would not doubt a word. And the 'refused to attend' 'not engaging' , letters after event etc..are all things Ive experienced too.

I had agreement from head of info governance last january that I cld have a copy of every appointment records with my care co-ordinator at next appointment and could have additions made as an edit to the contemporaneous record. So far, two copies out of numerous meetings.

I like your thinking. Interesting that they've failed to deliver, despite agreeing to it. Why, one wonders? Do you record your consultations?
