      I'm accustomed to (medical) mistakes now. I just assume that they'll be made, and am alert to look out for them; correct any significant ones that might affect my future care. Usually even those are impossible to get corrected in writing through correct channels; that's life...

      Vital information on my dying wife's "care plan". Despite that I dictated it, a "care co-ordinator" got it wrong twice before she grudgingly put it right! I was also accused of refusing to go to an MDT meeting that I was NOT notified of!

      That sounds distressing. Did you receive a letter AFTER the meeting had been held inviting, with a historical-date entered? (I was given a signed Bupa referral on 27th June 2013 DATED 10th September 2012. That was on GP file, rather than ACTUAL 21st September 2012 Bupa referral).

      That’s horrible, and genuinely insulting.. I’ve had comments made that I’ve ‘declined’ care that I was never offered; usually I find out (notes) that my previous GP declined it without involving me. Or even worse, it’s a CMA (cover my a’se) by a medic who failed to do their job.

      I wrote an article for the National Council for Palliative Care, about my wife's last few years. I have considered writing one about all thee shite that happened after her death but no bugger would believe it!

          Yeah would not doubt a word. And the 'refused to attend' 'not engaging' , letters after event etc..are all things Ive experienced too.

          I had agreement from head of info governance last january that I cld have a copy of every appointment records with my care co-ordinator at next appointment and could have additions made as an edit to the contemporaneous record. So far, two copies out of numerous meetings.
