So if I'm a clinician, what difference does that make? If I act against policy, is there a more serious consequence to that if I act against guidelines?

Yes. A guideline is just that, to be used to inform decision-making, the person should be able to justify their decision taken in the individual circumstance. Policy is dictated by a body/org and all members/employees/contractors must abide by it.
