We audit virtually everything we consider important and for CQC it is useful to demonstrate that we are at least trying to do things right even if not perfect and can show whether improving or not year on year

Varies slightly I suspect depending on whether it's guidance or policy.

So if I'm a clinician, what difference does that make? If I act against policy, is there a more serious consequence to that if I act against guidelines?

Yes. A guideline is just that, to be used to inform decision-making, the person should be able to justify their decision taken in the individual circumstance. Policy is dictated by a body/org and all members/employees/contractors must abide by it.
