With our son only one doctor wrote notes on the night. It was the junior/SHO and her notes matched our recollection - bless her. The other doctors didn't and were able to "make up" what happened with zero consequences.... so far.

There is an unwritten rule that applies there. If it wasn't written it didn't happen. Documented & recorded facts should always take precedence.

Yes but families don't typically (and shouldn't have to) keep contemporaneous notes/records and some recorded 'facts' aren't factual.

I advise anyone who'll listen to record every medical consultation now. Helps to revisit oft difficult to remember conversations, it's our legal right & also the only way to ensure accurate notes are recorded... as it turns out.https://twitter.com/c7rky/status/683662528224845825 …

I’m no saint, but I quite often write things down for people so they’ve got a written record of what we’ve agreed. Not just for people with dementia, but human beings who struggle to take in lots of new info in stressful situations. I think people find these “notes” helpful.

I sometimes say “I’m writing this down because your family might ask you later how it went with the Dr.... and you can show them this”. Patient also gets copy of any letters. We shouldn’t be scared of sharing info!

See... I knew there was a good reason why I followed you, Trisha. :) But also what Allyn said.

All doctors should read widely, but I particularly recommend
@sarasiobhan &@JamesTitcombe as good people to follow here. (Many others as well, you probably know some of them too).1 reply 2 retweets 2 likes -

Too many to list and certainly not worth the attempt, given the high risk of forgetting someone and unintentionally causing offence! I'll just say lots...
