David, can you say hand on heart that Dr's don't know of colleagues whose practice is poor yet do nothing about it? Of course some investigations will rightly side with clinicians but don't pretend that all investigations are candid!

I wasn't next of kin, but was main point of contact, after insisting, I was given a copy . The report was going to be used for learning lessons .

Ok, so you managed to get it eventually. That's something, I suppose. Hard to learn lessons if nobody's allowed to read it though!

John , our family buried two members of our family in 2010, the report was used for learning I understand by trust. Many bereaved families have different views and take different actions , or none, all should be respected and all choices are valid.

Agree,had enough evidence for me to request it was examined after deaths. We were right not to trust internal process. They play games even with evidence and witnesses. Their own report into our family members death was so confidential they didn't want us or MP to have copy.
