Because it’s in the notes, which means it definitely happened and is absolutely true, and that the patient/family were absolutely clear on the plan, even if they say they were not.

Absolutely. The only reason I got anywhere was because of my lifelong habit of taking contemporaneous notes at every meeting I attend. Like you, I couldn't possibly have appreciated the huge significance they would take on several years later.
