The idea is to not need an inquiry into avoidable deaths but have an ongoing process that investigates and learns on an ongoing basis. Leaves a lot of us so called ‘historic cases’ unresolved though. Anyone ever counted us up? Unresolved cases and number of years..

1/2 7th year now, but still not sure how to respond to this, because I never got chance to submit a formal complaint. Just 9 days from my very first email with the trust, they'd opened & closed a complaint I'd never made & spat the PHSO paragraph at me. Any advance on 9 days?

This was the point that I decided the internal complaints procedure was of no value to me, and that they could all answer for their actions to the police instead tbh. Still working on that...

I was told if a crime had been committed then go straight to the police .. I was at that time just a few days to the IR meeting.. I said, I'll wait 4the conclusion of this review & we are pleased we did.. hole got deeper now 18yrs its absolutely disgraceful but whocares?

the police sent me back to the NHS complaints system when I went to them - they told me they did not get involved in NHS complaints even where the law had been broken.

Told me same. Over & over. And when I proved them wrong, I had a memorable one on one meeting with a CH Insp who told me "If you want the police to investigate this, you're going to have to get the IPCC to *make* us do it." He was wrong about that too. An MP & QC did the trick.
