when i was at a recent 'learning from deaths" event several senior clinicians talked about all the new procedures they were putting in place/had put in place to review deaths, involve families etc and even as they were saying so got "so nothing has changed then?" not helpful

I attended

Think you at least deserve a reason. I was told I was not included on day2 due to capacity, it was primarily for trusts with a few patients. That’s fine, their choice, at least they were honest! But could follow online (and tried to but technology wasn’t great!)

I actually booked on line it was a shambles to be honest & had to chase.I was then asked did I want to attend day2 but I was not able.. also had an invite for December but it seems it was not everyone... dont know right hand from left

To be fair, I don’t think they expected what they got. 70 previously ignored, bereaved families with 00’s of years of experience of the system, And passion for change, must have been daunting.

Fair point. Though I bet it wasn't as daunting as it has been for those 70 families having to take on the entire medical establishment and being ignored. Everything's relative...

Indeed. Very true. We’re in it for the long run. I’ve just seen a few similar reviews now and every time, the staff start from a point of no understanding, through the process they seem to ‘get it,’ make the plans for change, until the next rime. It’s a mind numbing cycle

Send us round for another lap. And another... until we collapse or give up. Either will do. And with each passing lap more patients get harmed or killed because lessons go unlearned. Despite vexatious claims to the contrary. (See what I did there..?)
