      Our consent forms have a tear off that is given to the patient. Forms are sometimes changed prior to procedures when discussions with patient or team suggest need. Usually initialled. Personally never seen one altered after. = alteration of medical record. Bad plan.

      I was thinking the same. 21 years in #nhs never heard of a forged consent. Obviously happens. Consent gets changed after discussion with patient on some occasions.       Personally I don’t recognise those ruses but others may. I alter about 1 in 20 pre op. Juniors are better at thinking of and listing complications than me. Most of the real consent process 4 me occurs in clinic. Some minor repeat ops I verbally consent for only.

      I appreciate that Mike. Those who would never consider doing such a thing are always shocked at the mere suggestion. And you're right - even the law recognises it's the discussion, not the form, that counts. Forms matter to lawyers though.

          If I’d felt during the operation I’d varied something then the place to record it is in the notes not the consent form, along with full justification and mitigation. Lawyers will get you anyway, patients interest & honesty only option.
