Overseeing consultant surely has to take some responsibility? Why have a chain of command if there is no chain..

The Consultant who did not review the child who died may also have valid reasons. Apparently the listed Consultant on call was not in the UK at that time ....

Sounds like the bigger picture needs to be looked at again and all those involved to have a fair chance of representation. Family must be heartbroken. No one wants to see scapegoating.

A child died, from a possibly treatable illness. Nothing, absolutely nothing, can be as bad as that. But to then scapegoat one person and not learn the real lessons that needed to be learned is unforgivable. It compounds the initial failure, with another one nearly as bad.

So many families have said they want lessons to be learned. Its really important we learn and rectify. I agree with you.

I learnt most from my patients. My mistakes taught me some very hard lessons. Those I was most embarrassed about were the ones my patients forgave me for and from which I learnt most. Those I got sued for were minor and I learnt nothing, except it's annoying to be sued.

Sometimes, if you make a mistake, patients can see how much you are hurt by it and they are then very forgiving.

I'd have loved for us to have been given that choice. We'd have forgiven. No question - we're all human. There would've been no complaint, no lawyers, no police and treatment to correct life-threatening error would've been prompt. Alas, it was covered up.
