Independent review critical, I agree. Which brings us to the evidence. Somehow, immediate removal of medical records from trust custody after the incident is needed, imho. Only criminal investigations this would produce are for lack of candour or reckless acts...

Independent investigations vital, but could skills be passed on to risk managers. Will electronic notes make cover ups more difficult.

Electronic records have potential to help, yes. Depends on specific system. Risk managers, I'm not sure. They too would have to be independent though. What did you have in mind?

I’m not sure, but currently the majority of risk managers have no formal training, they admit this. A trained investigator could mentor.

I think I'd need to know more about the particular skills such a role brings in order to comment knowledgeably. Sounds like this has a part to play though.

Wow! That is one serious report. I'm so sorry Karen. This, I found particularly striking: "It is most unusual for any patient to actually give consent to investigate the very problem that caused death only nine days later after cessation of treatment for that condition."

Thankyou John It's heartbreaking can't understand senior coroner said he'd been a coroner over 12 years and has seen a lot worse pressure sores than mams several grade 4 pressure sores infected you can't get any worse than that so how many more avoidable deaths coroner's ignore?

The trouble is if they won't disclose the facts, you haven't got the facts with which to prosecute them - in our case, hospital says microbiology system has been changed and records on it gone. They have no record of doctor in charge of shift on ITU.

Yes, securing the medical evidence unquestionably has to be a part of this debate. Absolutely no immunity unless certainty of full disclosure can be assured for me.
