@C7RKY Targetted at frail elderly but we have had younger patients in their 50s for specific tasks.

@C7RKY Elderly often do badly in hospital & quickly become reconditioned so better for many if they stay at home & usual carers etc.
@Trisha_the_doc Sounds very good. Sort of thing my mum would probably appreciate too. Does it extend to EOL care?
@C7RKY Yes & no. We work in tandem with existing services, so if looks like eolc we get GP & District Nurses back in +/- palliative care.

@C7RKY Yes, we can arrange every test the hospital can, but many patients say "thanks doc, I'd rather not!"...

@C7RKY Dislike of transport, scared of hospitals, acknowledgement that tests won't necessarily change treatment plan.
