Of course views all mine. All without prejudice. Just a regular chap after all. Oh...and RT's may equally imply ridicule as endorsement.

@C7RKY I've never seen any that would be inappropriate for any party to read and I've seen hundreds

@Cleverestcookie But how does one judge what's appropriate? Not all records are true. May look A-ok on the surface, but pt may say different

@C7RKY I meant inappropriate phraseology. Problem may be with incorrect Read code selection, but wouldn't think widespread inaccurate

@Cleverestcookie I'll never be in a position to comment on how accurate records are on scale tbh. Can't look. Know of plenty that aren't.tho

@Cleverestcookie Now that, I like. :) I just wish the new IT systems didn't all seem to keep ducking electronic input for consent forms now.

@C7RKY how do you record patient's 'signature' if form is electronic though?

@Cleverestcookie You don't. Print it off for signature, then scan it back in. But important part is, input gets stored electronically then.

@Cleverestcookie Appreciated. My suggestion doesn't change that. It's about how you produce what you're going to scan. Paper, or electronic?
