@C7RKY my point is why do we HAVE to discuss and get agreement to decline a futile treatment? We don't HAVE to discuss any other futile RX? -
@doctorcaldwell Because people get a bit touchy if someone makes a decision that could end their life without consultation perhaps? -
@C7RKY this is why Gold Service Framework, Amber Care Bundle, Advanced Directives are so important. Failing to prepare is preparing to fail. -
@doctorcaldwell I'm with you on this one. I now have a power of attorney for my mum & a clear idea of her wishes. But fear drove me to do so -
@C7RKY not fear compassion. Neither of my parents wanted heroics. One died peacefully in a hospital one in nursing home. No drips just peace -
@doctorcaldwell Sorry - I missed this at the time. I can assure you it was fear in our case doc. Sad maybe, but absolutely true I'm afraid.

@C7RKY Pneumonia You will be home in 3 days in meantime if you have a cardiac arrest do you want CPR? Result one very confused frail person! -
@doctorcaldwell Well of course. But surely you'd tell them more than that? To explain logic that must exist behind your thinking at least? -
@C7RKY eg BreathingResus suction aspirated lunch out of airway in patient with MND. So in my sticker text "not for chest compressions" -
@doctorcaldwell There are others on here better qualified than I to comment on this tbh, but as a pt, I'd still expect the discussion now. -
@C7RKY John you would be easy to talk with and a decision respecting autonomy would be straightforward! :) -
@doctorcaldwell Ha! You know me so well doc...you said my magic word - autonomy. There's a tale to tell there, but respecting that is key. -
@C7RKY Autonomy the first principle of Medical Ethics http://en.m.wikipedia.org/wiki/Medical_ethics … "Medical Ethics" not heard much these days. All targets&money! -
@doctorcaldwell Quite! Used to quote the medical world as the perfect ethical business model, when I trained execs - feel a bit foolish now.

@C7RKY I'm with you on this one. Any advance decision must have patient agreement. Different if urgent unplanned clinical assessment -
@Cleverestcookie And there was me thinking the appeal court had made that point clear last week? Guess not clear enough yet..? -
@C7RKY I can see why they did it and it can be problematic in some circumstances but it can be managed by discussion with patient, adapted >
