@NHSComplaint It must be on the outcome, not the "suspicion". An unexpected death/maiming should be a trigger.

@C7RKY@nhscomplaint My wife went in healthy with a minor problem and ended up dead, that's unexpected in my book. -
@MartinBromiley@C7RKY@NHSComplaint SPOT ON Martin.....but is Unexpected death in NHS dictionary or does it need clarifying? -
@JoinedU1@c7rky@nhscomplaint It needs to be clear. Today I asked Boards in the NW to avoid %ages & use numbers and names killed/maimed -
@MartinBromiley@joinedu1@nhscomplaint I think that's a good call Martin. Harder for them to hide problems in the numbers & spin that way.

@C7RKY@MartinBromiley@NHSComplaint Error reporting system wd have pict up 0 incidents for aunt and about 6 for dad - wrong meds, fall etc -
@buletproofcardi@c7rky@nhscomplaint The errors alone plus the awful "unexpected outcome" should require investigation. Sorry to hear it.
