@JFr4ser the way in which complainants are crushed, medical records are 'weeded & seeded' & harm covered up, is also not random I'm afraid. -
@C7RKY I have no experience of that so can't comment. Records are often imprecise particular in very acute situations. Might be haggled over -
@JFr4ser Didn't see adding procedures to signed consent form post-op (to hide a crime) as 'imprecise' tbh. Nor when they presented a forgery -

@JFr4ser In normal circumstances, I'm afraid forging notes is only regulated through professional codes - GMC/NMC. Not a statutory offence.> -
