blaming 'the system' allows people who lead it, to escape accountability
@JFr4ser @heatherawwood @debhazeldine @joinedu1 @drgrumble @annclwyd I'm ever so glad - I was unsure if we would, for a brief moment! :-)

@C7RKY@heatherawwood@debhazeldine@joinedu1@drgrumble@annclwyd Covering up is unacceptable. Working out why & changing more difficult -
@JFr4ser Going to remove these poor folk from this chat to save their TLs >@heatherawwood@debhazeldine@joinedu1@drgrumble@annclwyd -
@C7RKY its always the way with these threads...ends up in loads of directions as others join in
-
@JFr4ser it's NOT pretty. I know about a lot of what goes on now. I'm beginning to understand the dynamics of why, but still learning. :) -
@C7RKY Join the club, just when I think I figure things out something new pops up
-
@JFr4ser The way the whole thing is set up is ridiculous from an outsider's perspective. Conflicting agendas everywhere, with lives at stake -
@C7RKY < mainly aspirational IMO -
@JFr4ser But I am hearing the 'burden of regulation' objection, as a reason that the safeguards aren't there. I don't understand why not yet - 14 more replies

@C7RKY@heatherawwood@debhazeldine@joinedu1@drgrumble@annclwyd < sometimes there's perception of poor care but usually grief response> -
@JFr4ser@c7rky@debhazeldine@joinedu I find it upsetting when you/staff attribute legit concern about how someone died to a grief response -
@heatherawwood < futile care wouldn't change it & would in itself be unethical to the pt! -
@JFr4ser What you say sometimes the case, but sadly so is rubbish care. Mid Staffs & Maidstone for ex, established that. People died unnec. -
@heatherawwood There is little evidence that Mid staffs is endemic throughout the NHS. Certainly not all aspects together in one org

@C7RKY@heatherawwood@debhazeldine@joinedu1@drgrumble@annclwyd < harm. The Swiss cheese effect as its known -
@JFr4ser@C7RKY@heatherawwood@JoinedU1@DrGrumble@AnnClwyd Disagree, poor care can result in harm. Believe me I know.#francis -
@DebHazeldine I know but it's not common -
-
@DebHazeldine Don't twist what I said. I said grief can make people believe there's been poor care when there hasn't been. -
@JFr4ser Not twisting anything. I speak of my personal experience, do not judge others experiences. -
@DebHazeldine In my experience poor care that harms is not common -
@JFr4ser@debhazeldine Maybe you work in good Hosp with low mortality, good care & outcomes. But lots not like that. Chaotic & high risk. - 8 more replies
