      @C7RKY Yes. Same here. The patient safety feedback fails. You can see why the system is failing if you observe it objectively.

      @C7RKY Presumably you have seen the NHSLA figures. Ask yourself how many confidentiality clauses prevent patient safety.

      @C7RKY There was a case once - the hospital had been negligent, the patient disabled. Oddly, the hospital "lost" the notes. Convenient?!

          @C7RKY You will find that medical record altering is common and non accountable. Powell v UK demonstrated this aspect.
