Of course views all mine. All without prejudice. Just a regular chap after all. Oh...and RT's may equally imply ridicule as endorsement.

No I think the plan was to make the reporter do nearly all the work of the investigator.

Daft idea. Medical investigators need to be trained professionals.

I did Investigator Training which took an afternoon! I imagine Training to be a police Investigator or Detective takes a bit longer? I have some understanding of complexity design and error but most participants had none .. but got signed up as Investigators

In general the incidents are investigated by staff who have left the frontline so no longer know the realities and may not command much respect..

My case is currently under investigation, five years after the error and negligence were discovered. What will be found is entirely in the hands of the investigators. I have met them, and have no reason to think that they lack integrity, tenacity and courage. I hope I'm right.

I do hope you are right. It'd be really good to know about investigations that have been honest, open, skillfull, timely etc and that have lead to best possible outcomes, albeit horrible circumstances, proper apologies, proper compensation,no cover ups. Anyone experienced this?

It seems to me that it's the patients who are doing all the learning. Since @UMCUtrecht allowed my cancer results to lie unseen for two years, I have learned a tremendous amount. But the "teaching" hospitals are still a long way behind on the learning curve.

I think you are right. Hopefully patients like you are spreading the word to future patients and there are HCWs who are listening to you. Social media might be a real force for good for this learning. I am emboldened by you.

And I draw strength from the patients and doctors in this thread who see the reality of what goes on in health services. Sometimes though, I feel we are changing healthcare one HCW and one patient at a time. It's a herculean task.

Yes, I agree. I do think changing things fromthe start, getting it into training and education would help but even then, so much learning goes on in clinical practice being taught by old timers with out dated views. Very hard to bring about change. A socialist gov could help.

Therein lies the risk of apprenticeship-style education...bad habits and cultures passed on, as well as many good. I think need to change views on who the apprentices’ masters are, to include wider range of people rather than just ‘seniors’ in professional solos.

Patients are the best teachers. We are the ones who learn the most when doctors and systems fail. Healthcare is set up to shrug off failure while doctors often adopt a "shit happens" approach to #PatientSafety

Never understood common attitudes encountered as student/Jnr Dr in hospital, essentially relegating patients to level of their diagnoses, to whom things happened, discussions were had, judgements made. I felt drawn into abyss, & felt ill. Added massively to stress levels

I can imagine. Like ... "Tell the appendix in Bed 4 she can go home", sort of treatment? Being reduced to a disease must also affect the outcomes of patients - who recovers when she's being talked down to and treated like an ignoramus?
