Indeed, and thank you for sharing on dropbox those screenshots of the UI. It's as if the designers don't realise that it's clinical time that is wasted, and their own product obstructs QA in healthcare. OTOH the myriad of incompatible silo systems ... Comment please

Exactly, I have never understood http://that.Systems I've used, clinicians are expected to judge how the incident poses a risk to the trust! And there is a weird sort of matrix about 'how serious' and 'how likely' that is v. rarely possible to guess at but guess one must.
